چکیده

Background Adherence to protective cardiovascular medications is of huge importance in the prevention of serious morbidity and mortality [1]. This study seeks to identify factors that influence adherence to the treatment of cardiovascular medications and to analyse the characteristics of the baseline non-adherence Irish cohort of the UMPIRE study. UMPIRE (“Use of a Multidrug Pill In Reducing cardiovascular Events”) was a prospective, randomised, open-label, blinded-endpoint (PROBE) clinical trial carried out in three locations in Europe (Ireland, The Netherlands and the UK) as well as India. The Irish cohort was a total of 333 participants (total trial 2,004 of which half in India). It had a key eligibility criterion of established cardiovascular disease (CVD) or an estimated 5year CVD risk of ≥15%. The results were present at the American Heart Association 2012 Scientific Sessions.

INterpreting the Processes of the UMPIRE Trial (INPUT): protocol for a qualitative process evaluation study of a fixed-dose combination (FDC) strategy to improve adherence to cardiovascular medications

INTRODUCTION This paper describes a planned process evaluation of the Use of a Multidrug Pill In Reducing Cardiovascular Events (UMPIRE) trial, one of several randomised clinical trials taking place globally to assess the potential of cardiovascular drugs as a fixed-dose combination (polypill) in cardiovascular disease prevention. A fixed-dose combination may be a promising strategy for promoti...

Process evaluation of the impact and acceptability of a polypill for prevention of cardiovascular disease.

IMPORTANCE The Use of a Multidrug Pill In Reducing cardiovascular Events (UMPIRE) trial has shown improved adherence with the use of a polypill strategy when compared with usual medications for cardiovascular disease (CVD) prevention. To advance from efficacy to impact, we need a better understanding of why and how such a strategy might be deployed in complex health systems. OBJECTIVE To unde...

Effects of a fixed-dose combination strategy on adherence and risk factors in patients with or at high risk of CVD: the UMPIRE randomized clinical trial.

IMPORTANCE Most patients with cardiovascular disease (CVD) do not take recommended medications long-term. The use of fixed-dose combinations (FDCs) improves adherence in several clinical areas. Previous trials of cardiovascular FDCs have assessed short-term effects compared with placebo or no treatment. OBJECTIVE To assess whether FDC delivery of aspirin, statin, and 2 blood pressure-lowering...
